If you prefer a paper copy, you are welcome to stop by the office to pick up your 4-H Club’s Financial Packet.
Office Hours: Monday–Friday, 8:00 AM – 4:00 PM
All forms should be completed by the 4-H Club head advisors and are due to the Extension Office (or emailed to burdsall.9@osu.edu) by 4:00 p.m. on March 1.

NEW – Ohio 4-H Statewide Insurance Policy
- Once Clermont County 4-H enrollment officially closes, each 4-H Club will receive an invoice based on the total number of enrolled Volunteers, Youth members, and Cloverbuds per the Ohio 4-H Statewide Insurance Policy.

Request for accommodations due to the Extension Office by May 1.
Club Fundraiser Request
Fundraiser requests are due 45 days prior to the event and must be approved by the Clermont County 4-H Committee. The 4-H Committee meets on the second Monday of each month.
Club Transfer Form
Requests are due by March 1.
Across County Lines Policy and Request Form
Requests for 4-H Membership Across County Lines are due by February 15.
